Diabetic and Cancer Restoration Program – 
Our Diabetic and Cancer Restoration Program is dedicated to supporting individuals diagnosed with diabetes, cancer, and other chronic conditions. Through a combination of educational resources and rehabilitative exercise, we empower participants to manage their health, enhance their well-being, and improve their overall quality of life.

This program equips patients with the knowledge and tools needed to navigate their illness with confidence, promoting both physical and emotional healing.

Physician Referral Required – 
Available by physician referral only, this program specifically addresses the unique challenges faced by those living with cancer. It offers a holistic approach that combines physical and nutritional education, emotional support, and additional services to support the healing process and foster long-term health.

As a 501(c)(3) Non-Profit Organization, we operate with the help of grants and donations. Our goal is to provide these services free of charge to the community, as long as funding is available. If funding becomes limited, we may offer a sliding scale payment option, based on a completed financial assistance form. Please note that we do not accept insurance.

HOW IT WORKS

1. Obtain a Restoration Clinic Prescription Form

You can get the Restoration Clinic Prescription Form in one of the following ways:

  • From a participating physician’s office

  • Directly from our facility

  • Download it from our website (available at the bottom of this page)

Want to get ahead on paperwork?
You may also download and complete the Medical Release Form from our website (available at the bottom of this page).

Please note: The Prescription Form must be submitted along with the updated Medical Release Form.

2. Complete the Prescription Form

  • The form must be filled out in its entirety by the physician’s office.

  • If the patient is diabetic, the most recent A1C level must be included.

3. Submit the Completed Form

Completed forms may be submitted to The Carroll Wellness Center by:

4. Processing the Referral

Once we receive the referral, the patient will be placed on a waiting list until an opening becomes available.

5. Meet Your Restoration Clinic Specialist

When an opening becomes available, a Restoration Clinic Specialist — a Certified Personal Trainer (CPT) experienced in working with individuals with chronic illness — will contact you to schedule your sessions.

6. Program Participation

Participants receive a 4-month membership, which includes:

  • An individualized exercise program

  • 8 weeks of supervised training

    • Two sessions per week with a Restoration Clinic Specialist

  • After the initial 8 weeks, participants will continue their exercise program independently for an additional 2 months

If you’d like to continue after your 4-month membership:

  • Pick up a Financial Assistance Program form at the front desk

  • Eligibility may qualify you for up to 50% off monthly or annual membership fees

Individuals with any form of controlled chronic disease may qualify, including but not limited to:

  • Diabetes (Type I & Type II)

  • Stroke

  • Heart Attack

  • Hypertension

  • Arthritis

  • Rheumatoid Arthritis

  • Multiple Sclerosis

  • Asthma

  • PCOS

  • COPD

  • Obesity

  • Osteoporosis

  • Lower A1C levels

  • Maintain and stabilize blood glucose and blood pressure

  • Increase strength, endurance, flexibility, and balance

  • Improve overall quality of life

  • Reduce pain and disease-related complications

  • Increase knowledge and confidence in disease management

  • Reduce anxiety and improve symptoms of depression

  • Pre- or post-surgery and/or treatments within the past 12 months
    → Scheduled as soon as an opening becomes available

  • Treatments completed more than 12 months ago
    → Placed on a waiting list until a spot becomes available

Any diagnosed form of cancer qualifies for participation in the Cancer Restoration Program at The Carroll Wellness Center.

SERVICES RECEIVED

Participants in the Cancer Restoration Program receive:

  • 4-month membership at The Carroll Wellness Center

  • One-on-one training with a Restoration Clinic Specialist

    • 8 weeks total (two sessions per week)

  • Individualized exercise programming, which may include:

    • Resistance Training

    • Cardiovascular Training

    • Aquatic Training

  • Nutrition guidance and education with a Certified Nutrition Coach

  • Monthly Cancer Support Groups

  • Massage Chair and Percussion Therapy

  • Lymphatic drainage exercises

  • Personal care support, including access to high-quality wigs

HOW IT HELPS

Participation in the Cancer Restoration Program may help:

  • Improve quality of sleep

  • Increase strength, endurance, flexibility, and balance

  • Reduce fatigue

  • Relieve nausea

  • Support immune system function

  • Reduce anxiety and improve symptoms of depression

  • Prevent or improve lymphedema

  • Improve overall quality of life during and after treatment

  • Support healthy weight management
